- Code Quality - Is it well-built? (clean, tested, maintainable)
Spec MUST pass before quality review begins.

- Never skip either review stage
- Never proceed with unfixed issues
- Never dispatch multiple implementation subagents in parallel
- Never let implementer self-review replace actual review
