diagnose-why-work-stopped

Investigate stalled or repeatedly recovering task trees in Paperclip using task states, run records, and recent changes. Produce an evidence-based cause analysis and a phased product-rule plan for approval, without changing code.

OriginalShow the original description

Compatibility entry point for failed, stalled, looping, or over-recovered Paperclip work. Use fix-failed-paperclip-run for the full diagnosis, regression, repair, real verification, and green-PR workflow; honor an explicitly requested diagnosis-only scope.

AI rating90/ 100See how to install

Good for

  • Finding the exact point where a task tree stopped
  • Investigating repeated continuation or recovery loops
  • Separating human blockers from work agents can resume
  • Planning general rules to prevent recurring stalls

Overview

What it does

diagnose-why-work-stopped traces parent tasks, child tasks, and dependencies in Paperclip to find where work stopped or recovery began repeating. It uses run records, comments, and state changes to explain the cause. It classifies work as needing human action, actionable by an agent, or already being handled. It also reviews recent related changes and documents rules to prevent recurrence, together with a phased plan.

When it helps

Use it when you need an explanation of why work stopped, or when recovery tasks keep multiplying. The plan must meet three conditions together: work that can proceed continues, only real blockers stop work, and infinite loops are prevented.

Example requests

  • “Explain where this group of tasks stopped and why, with supporting run records.”
  • “Investigate why recovery keeps adding more layers, and propose rules to prevent it happening again.”

Notes

The scope is diagnosis and planning; it does not change code. Read doc/execution-semantics.md before investigating. After plan approval, the work is divided into child tasks with assignees and dependencies. It respects access restrictions on other companies’ information and explicitly labels explanations based on inference.

This overview was written by AI (OpenAI Codex) from the public SKILL.md. Check the original below for the exact behavior.

Objective evaluation

AI rated this skill on four points using only the contents of its SKILL.md. Stars and popularity are not taken into account.

Clear instructionsIs it clear when to use it and what it does?
AGoodIt explicitly targets stalled or looping work and defines its output as cause analysis and planning without code changes.
Concrete stepsAre steps, inputs, outputs, and examples spelled out?
AGoodIt sequences contract review, stop-point investigation, related-change review, classification, and required plan-document contents.
SafetyAre sending data, deleting files, and running commands disclosed?
AGoodIt excludes code changes and explicitly forbids bypassing cross-company access restrictions after a 403 response.
Easy setupDoes it need few tools or API keys, and are they documented?
BPartly lackingIt requires the execution contract document and access to issues and run records, but provides no setup procedure.

The total counts each point as A = 25, B = 15, C = 5. Rated by AI (OpenAI Codex) as of Oct 20, 2026. Treat it as a guide and read the original before installing.

Diagnose Why Work Stopped

Use Fix Failed Paperclip Run. It owns the current workflow: find the first causal failure, reproduce it red, repair the shared contract, test second-order effects, verify the real workflow, and deliver a PR with passing checks. A normal request to fix stopped work continues through that workflow without a separate plan-approval gate.

If the user or calling workflow explicitly asks for diagnosis only or requires approval before implementation, perform the investigation and safety analysis within that scope. For example, a Terminal-Bench diagnosis child does not override the loop's board approval boundary. Keep the next action and its owner explicit.

For issue-tree diagnosis, read the current doc/execution-semantics.md, inspect the relevant subtree and recent repairs, and classify non-progressing nodes:

  • Genuine wait: a named human or external dependency must act.
  • Agent-actionable but unrouted: authorized useful work lacks a live path.
  • Already covered: existing work owns the next action; do not duplicate it.

Preserve these three invariants together:

  1. Productive work has a real next action and owner.
  2. Only genuine blockers stop work, including approval, credentials, and budget.
  3. Recovery is bounded and cannot recursively recover itself.

Frame any repair as a general behavioral contract, check it against a productive counterexample, and follow the new skill through the authorized finish line. Historical task links and private incident access are not required.
