Follow breast-screening guidelines for transgender females with no increased risk

STEP 1 — Gather Information

Collect patient gender identity, current age, duration and type of gender-affirming hormone therapy (estrogen ± anti-androgen), personal and family history of breast cancer, known genetic mutations (e.g., BRCA), prior chest radiation, and any prior breast surgery or mastectomy to determine baseline risk status.

STEP 2 — Rule In / Rule Out

If the patient is a transgender female and has been assessed as having no known increased risk of breast cancer (no personal/family history, no BRCA, no chest radiation, etc.), proceed to step 3; otherwise, follow high‑risk breast‑cancer screening pathways.

STEP 3 — Classify or Stratify

Stratify by age using average‑risk guidelines for cisgender females: ages 40‑49 (consider shared decision‑making for annual mammography), 50‑74 (biennial mammography), 75+ (continue if life expectancy ≥10 years). Adjust for hormone‑therapy‑induced breast density if needed but do not change interval.

STEP 4 — Decide

Recommend screening mammography according to the age‑based interval determined in step 3; if breast tissue is absent (e.g., after total mastectomy), discontinue routine screening and follow post‑mastectomy surveillance.

Clinical Guardrails / Mimics / Pitfalls

Do not withhold screening because of hormone therapy; do not apply male breast‑cancer guidelines; do not delay screening due to concerns about feminization; recognize that estrogen therapy may increase breast density but does not alter recommended screening interval; if a mastectomy has been performed, follow post‑mastectomy guidelines instead of average‑risk screening.

Concrete Clinical Example

A 52‑year‑old transgender female on estradiol 4 mg daily for 3 years reports no personal or family history of breast cancer, no BRCA mutation, and no prior chest radiation; she is assessed as no increased risk. Following step 3, she falls in the 50‑74 age bracket, so step 4 recommends biennial screening mammography.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
