jes-pa-bypass-confirmatory

Identifies patients with a positive aldosterone-to-renin ratio screening who may proceed directly to subtype testing without confirmatory testing based on specific clinical criteria. Triggers include spontaneous hypokalemia, baseline plasma aldosterone concentration >100 pg/mL (CLEIA), renin suppression, or a clinician questioning the need for a confirmatory test after a positive screen.

Determine when confirmatory testing can be bypassed for PA diagnosis

STEP 1 — Gather Information

Collect screening results: aldosterone-to-renin ratio (ARR) ≥200 (PAC/PRA) or ≥40 (PAC/ARC) with PAC ≥60 pg/mL (CLEIA); note spontaneous hypokalemia (serum K+ <3.5 mEq/L), baseline PAC >100 pg/mL (CLEIA), and renin suppression (PRA <0.5 ng/mL/h or ARC <2.5 pg/mL).

STEP 2 — Rule In / Rule Out

If the patient has a positive screening test AND exhibits spontaneous hypokalemia, baseline PAC >100 pg/mL (CLEIA), and renin suppression, rule in for bypassing confirmatory test; otherwise, rule out bypass and proceed to confirmatory testing.

STEP 3 — Classify or Stratify

Classify as eligible for confirmatory test bypass when all three criteria (spontaneous hypokalemia, high baseline PAC, renin suppression) are present; stratify as not eligible if any criterion is missing.

STEP 4 — Decide

Decide to skip confirmatory testing and move directly to subtype testing (e.g., adrenal venous sampling) for patients meeting all bypass criteria; for those not meeting criteria, order a confirmatory test (e.g., captopril challenge test or saline infusion test).

Clinical Guardrails / Mimics / Pitfalls

Do not bypass confirmatory testing in patients with borderline PAC (60–100 pg/mL CLEIA), normokalemia, or incomplete renin suppression; avoid mistaking diuretic-induced hypokalemia for spontaneous hypokalemia; remember that bypass does not exclude the need for subtype testing before adrenalectomy.

Concrete Clinical Example

A 45-year-old hypertensive patient presents with spontaneous hypokalemia (K+ 3.0 mEq/L), screening ARR 250 (PAC/PRA) with PAC 120 pg/mL (CLEIA), and PRA 0.2 ng/mL/h. All bypass criteria are met, so confirmatory testing is skipped and adrenal venous sampling is performed for subtype classification.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
