Monitor dental hygiene and oral health during antiresorptive therapy

STEP 1 — Gather Information

Collect medication details (type, dose, start date), baseline oral health status (recent dental exam, symptoms such as pain, swelling, loose teeth, exposed bone), cancer type, renal function, vitamin D and glucocorticoid use, and patient‑reported oral discomfort.

STEP 2 — Rule In / Rule Out

Determine if the patient is currently receiving or about to start an antiresorptive agent (IV bisphosphonate or denosumab). If no, follow routine dental care; if yes, proceed to next step.

STEP 3 — Classify or Stratify

Assess for signs/symptoms suggestive of osteonecrosis of the jaw (exposed bone, persistent pain, swelling, infection, non‑healing extraction site). Classify as: (a) asymptomatic, or (b) symptomatic/suspicious.

STEP 4 — Decide

For asymptomatic patients, schedule a routine visual dental exam every 6 months and reinforce oral hygiene instructions. For symptomatic/suspicious patients, refer promptly to a dentist or oral surgeon for evaluation, defer invasive dental procedures until assessed, and notify the oncology team.

Clinical Guardrails / Mimics / Pitfalls

Do not delay dental evaluation for new oral pain or exposed bone; avoid extractions, implants, or bone surgery without prior dental/oncology consultation; do not assume absence of symptoms rules out ONJ; do not neglect vitamin D and calcium monitoring which influence bone health.

Concrete Clinical Example

A 62‑year‑old woman with breast cancer–related HCM starts monthly denosumab. She reports mild gum soreness but no visible lesions. Step 1 confirms denosumab start and mild soreness. Step 2 confirms antiresorptive therapy. Step 3 classifies as symptomatic (oral discomfort). Step 4 leads to prompt dental referral; exam reveals early mucosal ulceration, no exposed bone; she receives improved oral hygiene instructions and follow‑up in 4 weeks.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
