登记、更新或清除前，必须先阅读并遵守：
登记、更新、清除都必须调用 ../../scripts/clinical_profile.py。不得直接编辑 USER.md，也不得自行寻找其他 USER.md。订阅任务创建不在本技能范围内，由 subscription-setup skill 负责。
只收集以下 5 项医生信息：
不得索取患者信息。不得把医院能力、设备条件、床位规模等问题变成强制登记表。
学习目标、学习诊断摘要和学习轨道可在用户确认后单独保存；它们不要求这 5 项信息。只有用户需要按地区、医院、科室、职称定制或开启订阅推送时才登记。
在保存登记信息前，必须获得医生对这 5 项信息处理的明确同意。若用户已经给出字段但没有明确同意，先要求确认，不得直接写入状态或生成回执。
登记信息通过档案脚本写入结构化状态，再由脚本自动生成 USER.md。如果脚本拒绝写入，不得向用户宣称登记已完成。不得把患者信息写入任何档案。

如果当前工作目录不是 skill 目录，使用专家包根目录下的脚本；不要传入文件路径参数。
登记可能跨多轮完成。每轮只问一个缺失字段，不要一次性列出完整表格，除非用户主动要求一次性补齐。

登记回执给出后，按用户意图分流（由 intent-routing skill 路由）：

登记回执使用 ../../references/output-templates.md 中的模板。只有 register 返回 ok: true 后，才能输出已记录。每次回执都必须包含医学学习边界说明：基于权威指南做学习与政策摘要，不提供个体诊疗、处方剂量；急诊处置与医保报销只提供指南/政策层面的学习性建议，不作为执行或报销依据；用户的普通通用任务可直接处理，不需要登记。
