ata-aed-lt4-monitoring

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Monitor free T4 and adjust levothyroxine after starting antiepileptic drugs in central hypothyroidism

STEP 1 — Gather Information

Collect current levothyroxine dose, date AED initiated, baseline free T4 (if available), recent free T4 result, and clinical symptoms of hypo‑ or hyperthyroidism. Confirm the AED is enzyme‑inducing (e.g., phenytoin, carbamazepine, oxcarbazepine, topiramate).

STEP 2 — Rule In / Rule Out

Is the patient on levothyroxine for central hypothyroidism and has recently started an enzyme‑inducing antiepileptic drug? If yes, proceed to Step 3; if no, this pathway does not apply.

STEP 3 — Classify or Stratify

At ≥6 weeks after AED initiation, measure free T4. Compare to the target range (mid‑to‑upper half of the laboratory reference range). If free T4 is below target → classify as needing dose increase; if within or above target → classify as adequate.

STEP 4 — Decide

If free T4 is below target, increase levothyroxine dose by 12.5–25 mcg increments and schedule repeat free T4 in 6–8 weeks. If free T4 is within target, maintain current dose and repeat monitoring per routine (every 6–12 months or as clinically indicated).

Clinical Guardrails / Mimics / Pitfalls

Do not use TSH to guide levothyroxine dosing in central hypothyroidism. Avoid over‑replacement; monitor for symptoms of hyperthyroidism (e.g., tachycardia, weight loss). Be aware that certain AEDs (e.g., phenytoin) may impair free T4 assay accuracy—use equilibrium dialysis or ultrafiltration methods if available. Do not delay the first free T4 check beyond 6 weeks after AED start.

Concrete Clinical Example

A 48‑year‑man with central hypothyroidism on levothyroxine 100 mcg daily (baseline free T4 1.1 ng/dL, target 0.9‑9‑1.7) starts carbamazepine 200 mg BID for seizures. At 6 weeks, free T4 is 0.7 ng/dL (below target). Levothyroxine is increased to 112.5 mcg daily. Free T4 is rechecked in 6 weeks and returns to 1.0 ng/dL.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.

In patients with adrenal insufficiency receiving dexamethasone replacement, the guideline suggests increasing the dexamethasone dose when enzyme-inducing antiepileptic drugs are co-administered. Trigger phrases include "AI patient on dexamethasone starts enzyme-inducing antiepileptic drug" or "AI patient on dexamethasone requiring enzyme-inducing AED".
