enda-annual-screening-autoimmune-diseases

Suggests periodic screening for autoimmune diseases known to be more prevalent in PAI patients in whom autoimmune origin of PAI has not been excluded; optimal frequency unknown but can be done annually. Use when determining screening schedule for PAI patient with uncertain autoimmune origin.

Screen Annually for Autoimmune Diseases More Prevalent in PAI Patients

STEP 1 — Gather Information

Confirm diagnosis of primary adrenal insufficiency (PAI) and assess whether autoimmune origin has been excluded (e.g., 21-hydroxylase antibody testing). Collect patient age, sex, and any known autoimmune comorbidities. If autoimmune serology status is unknown, order 21-hydroxylase antibody test.

STEP 2 — Rule In / Rule Out

Is autoimmune origin of PAI excluded by negative 21-hydroxylase antibodies?

  • Yes: Stop; annual autoimmune screening per this guideline not indicated.
  • No or unknown: Proceed to step 3.

STEP 3 — Classify or Stratify

Classify patient as requiring annual screening for autoimmune conditions more prevalent in PAI with uncertain autoimmune origin: thyroid disease, type 1 diabetes mellitus, premature ovarian failure, celiac disease, and autoimmune gastritis with vitamin B12 deficiency.

STEP 4 — Decide

Order annual screening labs: TSH and free T4 for thyroid disease; HbA1c for diabetes; serum vitamin B12 and anti-parietal cell antibody (if B12 low) for autoimmune gastritis; anti-tissue transglutaminase IgA with total IgA for celiac disease; and consider FSH/LH or AMH for premature ovarian failure in appropriate patients.

Clinical Guardrails / Mimics / Pitfalls

Do not screen if autoimmune origin definitively excluded; do not rely on symptoms alone to trigger screening; avoid unnecessary repeat testing within 12 months unless clinically indicated; remember that pregnancy may alter interpretation of thyroid and glucose tests; consider HLA-DQ2/DQ8 genotyping if serology equivocal for celiac.

Concrete Clinical Example

A 34‑year‑old woman with PAI of uncertain autoimmune origin (21‑hydroxylase antibody not sent) presents for routine follow‑up. She has no autoimmune symptoms. Step 1 confirms PAI and unknown serology. Step 2 finds autoimmune origin not excluded. Step 3 classifies her for annual screening. Step 4 orders TSH, free T4, HbA1c, vitamin B12, anti‑tTG IgA, and anti‑parietal cell antibody; all return negative, and screening is repeated annually.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
