enda-glucocorticoid-therapy-all-pai

Recommends initiating glucocorticoid therapy in all patients with confirmed primary adrenal insufficiency (PAI). Use when PAI diagnosis is confirmed by subnormal cortisol response to ACTH stimulation test or elevated ACTH with low cortisol.

Initiate Glucocorticoid Therapy for All Confirmed PAI Patients

STEP 1 — Gather Information

Collect morning cortisol, plasma ACTH, and perform a 250 µg ACTH stimulation test; assess electrolytes, symptoms (fatigue, hypotension, hyperpigmentation), and review for mineralocorticoid deficiency.
Action: Confirm PAI diagnosis via subnormal cortisol response (<500 nmol/L) to 250 µg ACTH or elevated ACTH with low cortisol.

STEP 2 — Rule In / Rule Out

Is PAI confirmed by biochemical testing?

  • Yes: Proceed to Step 3.
  • No: Consider alternative diagnoses; do not initiate glucocorticoid therapy for PAI.

STEP 3 — Classify or Stratify

Classify patient as adult or pediatric to determine appropriate glucocorticoid dosing regimen.
Action: Select adult or pediatric dosing pathway.

STEP 4 — Decide

Initiate glucocorticoid replacement: adults hydrocortisone 15–25 mg/day or cortisone acetate 20–35 mg/day in two to three divided doses; children hydrocortisone 8 mg/m²/day in three to four divided doses. Add fludrocortisone if mineralocorticoid deficient, educate on stress dosing, and provide emergency steroid card.
Action: Prescribe replacement regimen and arrange follow‑up.

Clinical Guardrails / Mimics / Pitfalls

Do not use dexamethasone due to risk of Cushingoid side effects; avoid relying solely on hormonal levels to adjust dose—use clinical assessment (weight, blood pressure, energy). Ensure mineralocorticoid replacement when needed and provide steroid emergency card with clear stress‑dosing instructions.

Concrete Clinical Example

A 50‑year‑old man with confirmed PAI (ACTH stimulation test peak cortisol 300 nmol/L, ACTH nmol/L, ACTH 150 pmol/L) started on hydrocortisone 20 mg daily (10 mg on waking, 5 mg early afternoon, 5 mg late afternoon) plus fludrocortisone 100 µg daily; received steroid card and instructed to double dose during fever >38 °C.

概要と使いどころ

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
