enda-prednisolone-alternative-glucocorticoid

Suggests using prednisolone 3–5 mg/d orally once or twice daily as a glucocorticoid alternative in primary adrenal insufficiency. Consider this option when evaluating glucocorticoid therapy, particularly in patients with reduced medication compliance.

Use Prednisolone as Glucocorticoid Alternative with Specific Dosing

STEP 1 — Gather Information

Confirm diagnosis of primary adrenal insufficiency via ACTH stimulation test; assess current glucocorticoid regimen and adherence; evaluate need for mineralocorticoid replacement; review for pregnancy or other special considerations. If PAI is confirmed and glucocorticoid replacement is indicated, proceed to assess compliance.

STEP 2 — Rule In / Rule Out

Is the patient non‑adherent or experiencing reduced compliance with multiple‑dose glucocorticoid therapy? If yes, proceed to consider prednisolone; if no, continue with standard hydrocortisone regimen.

STEP 3 — Classify or Stratify

Select prednisolone dose 3–5 mg/d; choose once daily for simplicity or twice daily for more stable levels based on patient preference and lifestyle.

STEP 4 — Decide

Prescribe prednisolone 3–5 mg/d orally once or twice daily; ensure adequate fludrocortisone replacement if mineralocorticoid deficiency present; educate on adherence and stress dosing; schedule follow‑up in 4 weeks.

Clinical Guardrails / Mimics / Pitfalls

Prednisolone has reduced mineralocorticoid activity, so may require increased fludrocortisone; avoid in children (synthetic long‑acting glucocorticoids not recommended); do not use as sole glucocorticoid if mineralocorticoid replacement is inadequate; monitor for Cushingoid signs (weight gain, insomnia, edema); avoid dexamethasone due to higher Cushingoid risk.

Concrete Clinical Example

A 45‑year‑old woman with PAI on hydrocortisone 20 mg BID reports frequent missed doses; switched to prednisolone 4 mg once daily with fludrocortisone 100 µg daily; adherence improved and symptoms stable at 4‑week follow‑up.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
