endo-assess-dka-prevention-treatment

Assesses patient’s knowledge of diabetic ketoacidosis prevention, recognition, and treatment. Triggered by clinician prompts such as 'Review sick-day rules and DKA signs' or 'Ensure patient knows when to seek help for hyperglycemia'.

Assess DKA prevention and treatment knowledge

STEP 1 — Gather Information

Collect patient’s understanding of sick-day rules, ability to recognize DKA symptoms (nausea, vomiting, abdominal pain, fruity breath, polyuria, polydipsia), knowledge of when to check blood ketones, insulin adjustment strategies during illness, awareness of emergency supplies (ketone strips, glucagon, contact info), and ability to operate/troubleshoot the insulin pump.

STEP 2 — Rule In / Rule Out

Does the patient demonstrate correct knowledge of DKA prevention and treatment?

  • Yes → Proceed to Step 3 (adequate knowledge).
  • No → Proceed to Step 3 (inadequate knowledge).

STEP 3 — Classify or Stratify

Stratify inadequate knowledge into:

  • Partial: knows some elements (e.g., recognizes hyperglycemia) but misses key actions (ketone testing, insulin adjustment).
  • Deficient: lacks core knowledge (does not know sick-day rules, when to seek help, or how to adjust insulin).

STEP 4 — Decide

  • Adequate: reinforce education at next visit and schedule routine review.
  • Partial: provide targeted teaching on missing elements (e.g., ketone testing, sick-day plan) and confirm understanding.
  • Deficient: arrange urgent diabetes educator referral, supply written sick-day plan with ketone testing instructions, ensure emergency supplies are available, consider temporary MDI backup, and schedule follow‑up within 48 hours.

Clinical Guardrails / Mimics / Pitfalls

Do not assume knowledge based on duration of diabetes or pump use; avoid overlooking cognitive impairment, health literacy, or reliance on self‑report without demonstration; do not neglect to confirm the patient knows when to seek emergency care (e.g., persistent vomiting, inability to keep fluids down, ketones moderate/large); do not forget to assess ability to troubleshoot pump failures during illness.

Concrete Clinical Example

A 22‑year‑old with T1DM on CSII presents with 2 days of vomiting and hyperglycemia (BG 350 mg/dL). Patient reports never checking ketones, unaware of sick‑day rules, and lacks ketone strips. Assessment reveals deficient knowledge. Educator provides a sick‑day plan, prescribes ketone strips, arranges diabetes educator visit within 24 hours, and ensures emergency supplies are stocked.

概要と使いどころ

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
