endo-cgm-confirmatory-pocbg-high-risk-hypoglycemia

Recommends real-time CGM with confirmatory bedside POC-BG monitoring for insulin dosing adjustments rather than POC-BG testing alone in adults with insulin-treated diabetes at high risk of hypoglycemia. Consider this approach for patients aged ≥65 years, BMI ≤27 kg/m², total daily insulin dose ≥0.6 units/kg, CKD stage ≥3, or recent hypoglycemia.

Use CGM with confirmatory POC-BG monitoring for insulin adjustments in high-risk hypoglycemia patients

STEP 1 — Gather Information

Collect age, BMI, total daily insulin dose (units/kg), CKD stage (eGFR), recent hypoglycemia history, and confirm insulin-treated diabetes with noncritical illness hospitalization.

STEP 2 — Rule In / Rule Out

If any of the following are present: age ≥65 years, BMI ≤27 kg/m², total daily insulin dose ≥0.6 units/kg, CKD stage ≥3 (eGFR <60 mL/min/1.73m²), or documented recent hypoglycemia → classify as high risk; otherwise, classify as not high risk.

STEP 3 — Classify or Stratify

If high risk, evaluate hospital availability of CGM devices, training, and protocols; if resources/training are available → select CGM with confirmatory POC-BG monitoring; if not available → select POC-BG testing alone.

STEP 4 — Decide

Implement the selected monitoring strategy to guide insulin dosing adjustments; document glucose trends and adjust insulin accordingly.

Clinical Guardrails / Mimics / Pitfalls

Do not rely solely on CGM without confirmatory POC-BG for insulin dosing; avoid CGM in patients with extensive skin infections, hypoperfusion, hypovolemia, or receiving vasoactive/pressor therapy; beware of medications causing CGM interference (acetaminophen >4 g/day, dopamine, vitamin C, hydroxyurea); ensure CGM resources and training are available before deployment.

Concrete Clinical Example

A 70-year-old woman with type 2 diabetes on basal-bolus insulin admitted for pneumonia (noncritical illness) has age 70, BMI 24, TDD 0.8 units/kg, CKD stage 3b; hospital has CGM resources. Real-time CGM with confirmatory POC-BG is initiated; a trend down triggers a confirmatory POC-BG of 68 mg/dL, leading to basal insulin hold and carbohydrate intake, preventing severe hypoglycemia.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
