endo-insulin-pump-continuation-vs-transition

Recommends continuing insulin pump therapy in hospitals with pump expertise; otherwise transitioning to scheduled subcutaneous basal bolus insulin if anticipated length of stay exceeds 1–2 days before pump discontinuation. Consider this when anticipating a hospital stay >2 days, lacking pump expertise, or needing to conserve supplies.

Continue insulin pump therapy or transition to subcutaneous basal bolus insulin based on expertise and anticipated length of stay

STEP 1 — Gather Information

Confirm pre-admission insulin pump use, assess hospital pump expertise, anticipated LOS, patient ability to self-manage (consciousness, ability to adjust settings), pump supply availability, and contraindications (critical illness, DKA, HHS). If any contraindication exists, transition to subcutaneous basal bolus insulin (BBI) immediately.

STEP 2 — Rule In / Rule Out

Is there hospital personnel with expertise in insulin pump therapy? If yes, proceed to Step 3; if no, proceed to Step 3 to evaluate LOS.

STEP 3 — Classify or Stratify

If anticipated LOS > 1–2 days, plan transition to scheduled subcutaneous BBI before pump discontinuation; if anticipated LOS ≤ 1–2 days and supplies are available, continuation may be considered.

STEP 4 — Decide

Continue pump with verification of supply adequacy and basal rate adjustment at admission; or transition to scheduled subcutaneous BBI using Table 3 conversion (basal = total daily basal rate, prandial/correction = weight-based fixed doses) and discontinue pump.

Clinical Guardrails / Mimics / Pitfalls

Do not continue pump if patient cannot self-manage, lacks supplies, has impaired consciousness, critical illness, DKA/HHS, or if anticipated LOS >2 days without expertise; avoid pump continuation solely to conserve supplies without safety checks.

Concrete Clinical Example

A 58-year-old with type 1 diabetes on insulin pump admitted for elective cholecystectomy with anticipated LOS 3 days; hospital lacks pump expertise, patient is alert and able to adjust settings, supplies available. Transition to scheduled subcutaneous basal bolus insulin before pump discontinuation per guideline.

概要と使いどころ

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
