Recommends RT-CGM devices for adult patients with well-controlled type 1 diabetes who are willing and able to use the devices on a nearly daily basis. Trigger phrases include 'Controlled T1DM patient interested in CGM' and 'Consider RT-CGM for glycemic stability in well-controlled T1DM'.

Recommend RT-CGM for well-controlled T1DM

STEP 1 — Gather Information

Collect patient age, diabetes type, most recent A1C, willingness and ability to wear RT-CGM sensor nearly daily, history of skin adhesives allergies, current glucose monitoring frequency, hypoglycemia awareness, and any psychosocial or financial barriers.
Action: Document A1C, device willingness/ability, and contraindications.

STEP 2 — Rule In / Rule Out

If the patient is an adult with type 1 diabetes, A1C ≤7.0%, expresses willingness and ability to use RT-CGM on a nearly daily basis, and has no contraindications (e.g., inability to wear sensor, lack of training capacity), proceed to Step 3; otherwise, do not recommend RT-CGM.
Decision: Proceed to classification or decline recommendation.

STEP 3 — Classify or Stratify

Classify the patient as having well-controlled T1DM (A1C ≤7.0%) and eligible for RT-CGM recommendation based on the gathered information.
Action: Confirm eligibility for RT-CGM prescription.

STEP 4 — Decide

Prescribe an RT-CGM device (e.g., Dexcom G6, Medtronic Guardian Connect), provide device-specific education on sensor insertion, calibration with SMBG, alarm settings, and data interpretation, and schedule a follow-up visit in 4 weeks to review glycemic data and device tolerance.
Action: Initiate RT-CGM therapy and arrange early follow-up.

Clinical Guardrails / Mimics / Pitfalls

Do not recommend RT-CGM for patients unwilling to perform occasional SMBG for calibration, those with unrealistic expectations of eliminating fingersticks, individuals with significant psychosocial distress that may impede device use, or patients lacking insurance/financial coverage for supplies. Avoid using RT-CGM as a sole basis for acute treatment decisions without confirmatory fingerstick glucose when symptoms do not match readings.
Pitfall: Overreliance on CGM trends without verifying hypoglycemia can lead to missed severe hypoglycemia.

Concrete Clinical Example

A 34‑year‑old woman with T1DM, A1C 6.8% on MDI, expresses interest in CGM to reduce hypoglycemia anxiety, confirms willingness to wear sensor daily, denies skin allergies, and performs SMBG 4 times/day. She meets criteria; RT‑CGM is prescribed, training provided, and follow‑up scheduled in 4 weeks.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
