Recommends real-time CGM with confirmatory POC-BG for insulin dosing adjustments in insulin-treated hospitalized adults at high risk of hypoglycemia when resources and training are available. Triggers include: "Should we use CGM for insulin adjustments?" or "Is CGM appropriate for this patient?

Decide on CGM Use with Confirmatory POC-BG

STEP 1 — Gather Information

  • Confirm insulin-treated diabetes (type 1 or type 2) and hospitalization for noncritical illness.
  • Assess high‑risk hypoglycemia criteria: age ≥65 y, BMI ≤27 kg/m², total daily insulin dose ≥0.6 U/kg, history of stage ≥3 CKD, liver failure, cerebrovascular accident, active malignancy, pancreatic disorder, CHF, infection, prior hypoglycemia, or impaired hypoglycemia awareness.
  • Verify availability of CGM resources (devices, supplies) and staff training for device use, data interpretation, and protocol adherence.

STEP 2 — Rule In / Rule Out

  • Is the patient at high risk for hypoglycemia?
    • No → Use POC‑BG monitoring alone for insulin adjustments (do not initiate CGM).
    • Yes → Proceed to evaluate resource availability.

STEP 3 — Classify or Stratify

  • Are CGM resources and training available in the hospital setting?
    • No → Continue with POC‑BG monitoring alone (CGM not feasible).
    • Yes → Proceed to decision step.

STEP 4 — Decide

  • If high‑risk hypoglycemia and resources/training available → Initiate real‑time CGM with confirmatory POC‑BG before any insulin‑dose adjustment.
  • Otherwise → Rely on POC‑BG monitoring alone for insulin dosing.

Clinical Guardrails / Mimics / Pitfalls

  • Do not rely on CGM alone for insulin decisions; always confirm with POC‑BG, especially in hypoglycemic range (<70 mg/dL).
  • Avoid CGM in patients with extensive skin infections, hypoperfusion, hypovolemia, or receiving vasoactive/pressor agents.
  • Be aware of substances that cause false CGM readings: acetaminophen >4 g/day, dopamine, vitamin C, hydroxyurea.
  • Do not use CGM if the patient cannot safely manage device alarms or if sensor placement is contraindicated.

Concrete Clinical Example

A 68‑year‑old woman with type 2 diabetes on basal‑bolus insulin (total daily dose 0.8 U/kg) is admitted for pneumonia. She is 68 y, BMI 26 kg/m², and had a hypoglycemic episode last month. The ward has CGM devices and trained nurses. She meets high‑risk criteria and resources are available, so real‑time CGM is started with POC‑BG confirmation before each insulin adjustment.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
