Recommends CGM over SMBG for type 1 diabetes patients on multiple daily injections to reduce hypoglycemia risk. Triggered by clinician questions such as "Should I use CGM or SMBG for this T1D patient on MDI?" or "Is CGM preferred for glucose monitoring in this patient?"

CGM vs SMBG for Type 1 Diabetes on MDI

STEP 1 — Gather Information

Confirm type 1 diabetes diagnosis and current multiple daily injection (MDI) regimen; assess hypoglycemia history (frequency, severity, nocturnal events), hypoglycemia awareness, patient’s ability/willingness to use CGM (dexterity, visual/hearing for alerts), insurance coverage/access, and need for structured education on device use and data interpretation.

STEP 2 — Rule In / Rule Out

Rule out CGM if the patient cannot safely operate the device (e.g., severe dexterity impairment, uncontrolled dermatitis, lack of training resources, or explicit refusal); otherwise rule in for CGM consideration.

STEP 3 — Classify or Stratify

Stratify by hypoglycemia risk: high (history of severe hypoglycemia requiring assistance, hypoglycemia unawareness, frequent nocturnal events) versus moderate (no severe events but suboptimal glycemic control); prioritize CGM for high‑risk patients.

STEP 4 — Decide

Recommend initiating real‑time CGM (or intermittently scanned CGM with predictive alerts) rather than SMBG alone; provide structured education on device use, troubleshooting, and data interpretation; retain access to SMBG for calibration/validation and sensor warm‑up periods.

Clinical Guardrails / Mimics / Pitfalls

Do not rely solely on CGM values for insulin dosing without confirmatory SMBG during sensor warm‑up, when symptoms mismatch sensor readings, or if sensor malfunctions; do not omit structured education; do not use CGM in patients with substances that interfere with sensor accuracy (e.g., high‑dose vitamin C, hydroxyurea) without verification; do not assume CGM eliminates the need for SMBG entirely.

Concrete Clinical Example

A 22‑year‑old college student with T1D on MDI (glargine basal, lispro bolus) reports two nocturnal hypoglycemia episodes requiring assistance in the past month and fears hypoglycemia disrupting sleep; after confirming MDI regimen and hypoglycemia risk, CGM is initiated with education, and SMBG is retained for calibration and symptom‑sensor mismatches.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
