es-check-adolescent-gnrh-eligibility

Determines if adolescent meets criteria for GnRH agonist treatment to suppress pubertal development. Triggers include adolescent requesting to suppress puberty, adolescent with gender dysphoria seeking pubertal blockade, or clinician noting Tanner G2/B2 with request for hormone suppression.

Check eligibility for adolescent GnRH agonist treatment (pubertal suppression)

STEP 1 — Gather Information

Collect: adolescent’s gender identity and distress, onset/timing of puberty (Tanner stage), mental health evaluation (duration/intensity of gender nonconformity, worsening with puberty), coexisting psychological/medical/social problems, adolescent’s capacity to give informed consent, parental/guardian consent, information provided about treatment effects (including fertility risks) and preservation options, pediatric endocrinologist assessment of pubertal status and contraindications.

STEP 2 — Rule In / Rule Out

Has a qualified mental health professional confirmed a long-lasting and intense pattern of gender nonconformity or gender dysphoria (whether suppressed or expressed) and that gender dysphoria worsened with the onset of puberty?

  • If No → Rule out: adolescent does not meet core diagnostic criterion for GnRH agonist treatment; address mental health needs and reassess later.
  • If Yes → proceed to Step 3.

STEP 3 — Classify or Stratify

Assess: (a) adolescent has sufficient mental capacity to give informed consent to this reversible treatment; (b) any coexisting psychological, medical, or social problems that could interfere with treatment have been addressed such that the adolescent’s situation and functioning are stable; (c) parents/guardians have consented and are involved in supporting the adolescent; (d) pediatric endocrinologist confirms puberty has started (Tanner stage G2/B2) and there are no medical contraindications to GnRH agonist therapy.

  • If all criteria are met → proceed to Step 4.
  • If any criterion is unmet → Rule out: address the specific gap (e.g., optimize mental health, obtain consent, treat comorbidities, await Tanner G2/B2) and reassess.

STEP 4 — Decide

If eligible → initiate GnRH agonist therapy (e.g., leuprolide depot 3.75 mg SC monthly or 11.25 mg q3mo) after confirming adolescent assent and parental consent, provide fertility preservation counseling, and schedule baseline monitoring (LH/FSH, sex steroids, bone density if indicated) per guideline. If not eligible → do not start GnRH agonist; instead, provide supportive care, address outstanding issues, and re‑evaluate when criteria are satisfied.

Clinical Guardrails / Mimics / Pitfalls

  • Do not start GnRH agonist before Tanner G2/B2 (prepubertal) as efficacy and safety data are lacking.
  • Do not proceed without documented informed consent from adolescent and parents/guardians, especially if adolescent < legal medical consent age.
  • Do not ignore untreated major psychiatric conditions (e.g., active psychosis, uncontrolled depression) that could impair adherence or decision‑making.
  • Do not omit discussion of fertility preservation options and potential loss of fertility if subsequent sex hormone treatment is pursued.
  • Do not use GnRH agonist as a sole intervention; ensure multidisciplinary team (mental health, endocrinology, primary care) involvement.
  • Monitor for bone mineral density changes and consider calcium/vitamin D supplementation per local protocols.

Concrete Clinical Example

A 14‑year‑old assigned female at birth presents requesting to stop breast development and menses. She reports worsening gender dysphoria with breast budding (Tanner G2). Her therapist confirms a 6‑month history of persistent gender dysphoria worsening with puberty, no major comorbidities, and adequate capacity to consent. Parents are supportive and have consented. Pediatric endocrinologist confirms Tanner G2/B2 and finds no medical contraindications. She has been informed of GnRH effects, fertility risks, and options; she assents and parents consent. Action: start leuprolide 3.75 mg SC monthly, schedule LH/FSH and estradiol in 1‑month, bone density baseline if risk factors present.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
