es-cushing-measure-serum-cortisol-ufc-off-medication-intervals

This skill recommends measuring serum cortisol or urine free cortisol off-medication at 6- to 12-month intervals to assess radiation therapy effect in patients with Cushing's syndrome undergoing or post-radiotherapy. It is also indicated when patients on stable medical therapy develop new adrenal insufficiency symptoms such as fatigue, weight loss, or hypotension.

Measuring Serum Cortisol or Urine Free Cortisol Off-Medication at 6- to 12-Month Intervals to Assess Radiation Therapy Effect and New Adrenal Insufficiency Symptoms

STEP 1 — Gather Information

Collect diagnosis of Cushing's disease, history of pituitary radiotherapy (type and date), current cortisol‑lowering medications and doses, timing since radiotherapy, symptoms suggestive of adrenal insufficiency (fatigue, weight loss, hypotension, hyponatremia, hyperkalemia), baseline serum cortisol or UFC values if available. → Assess whether the patient can safely discontinue medication for off‑medication testing.

STEP 2 — Rule In / Rule Out

Determine if stopping the current medical therapy is safe (e.g., hold short‑acting agents ≥24 h, longer‑acting per drug half‑life; ensure no recent adrenal crisis or uncontrolled hypercortisolism). If safe → proceed to obtain off‑medication serum cortisol or UFC; if not safe → do not obtain off‑medication sample and consider alternative assessment (e.g., clinical evaluation only).

STEP 3 — Classify or Stratify

Interpret the off‑medication serum cortisol or UFC using the assay’s normal range: normal → radiation therapy effective and no adrenal insufficiency; elevated → persistent hypercortisolism or inadequate radiation effect; low → new adrenal insufficiency. → Classify the result as normal, elevated, or low.

STEP 4 — Decide

If normal: continue current follow‑up and repeat testing at 6‑ to 12‑month intervals. If elevated: consider intensifying radiation therapy, adding or adjusting medical therapy, or evaluating for surgical revision. If low: initiate physiologic glucocorticoid replacement (hydrocortisone 15‑20 mg/day divided), provide stress‑dosing education, and monitor for recovery of the HPA axis.

Clinical Guardrails / Mimics / Pitfalls

Do not perform off‑medication testing in patients with recent adrenal insufficiency, unstable hypercortisolism, or life‑threatening symptoms; avoid interpreting a single abnormal result without repeat confirmation; do not adjust medication based solely on lab values without correlating with symptoms; be aware of assay variability and timing of sample collection; do not delay empiric glucocorticoid replacement in symptomatic adrenal insufficiency awaiting results.

Concrete Clinical Example

A 48‑year‑old man with Cushing's disease received stereotactic radiosurgery 10 months ago and remains on ketoconazole 800 mg/day. He reports progressive fatigue and orthostatic dizziness. Ketoconazole is held for 24 h; off‑medication morning serum cortisol is 2.2 µg/dL (below normal) and UFC is low. He is diagnosed with new adrenal insufficiency, started on hydrocortisone 15 mg morning/10 mg evening, and symptoms improve.

概要と使いどころ

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
