es-cushing-postop-pituitary-mri-within-1-3-months

This skill recommends obtaining a postoperative pituitary MRI within 1–3 months after successful transsphenoidal surgery (TSS) to establish a new baseline for future recurrence assessment. Use when a Cushing’s syndrome patient has achieved biochemical remission post‑TSS and is entering routine follow‑up.

Obtaining Postoperative Pituitary MRI Scan Within 1-3 Months After Successful TSS

STEP 1 — Gather Information

Collect postoperative day 5–7 urine free cortisol (UFC) or late-night salivary cortisol, morning serum cortisol, clinical assessment of remission (no symptoms of hypercortisolism), and date of successful transsphenoidal surgery (TSS). Confirm biochemical remission (e.g., UFC < upper limit of normal or morning cortisol <5 µg/dL) and stable clinical status.

STEP 2 — Rule In / Rule Out

Is the patient in biochemical remission after TSS? If yes, proceed to timing assessment; if no, evaluate for persistent disease and consider additional treatment per guideline 4.2.

STEP 3 — Classify or Stratify

Determine time since TSS: <1 month (early postoperative), 1–3 months (optimal window), or >3 months (delayed). If within 1–3 months, schedule MRI now; if <1 month, defer until 1 month; if >3 months, obtain MRI as soon as feasible and note delayed baseline.

STEP 4 — Decide

Obtain a dedicated pituitary MRI protocol with contrast (unless contraindicated) to establish a new postoperative baseline; document findings for future recurrence comparison.

Clinical Guardrails / Mimics / Pitfalls

Do not obtain MRI before 1 month postoperative to avoid mistaking postoperative blood or edema for residual tumor; do not rely solely on MRI for recurrence—combine with biochemical testing; avoid delaying beyond 3 months without justification as baseline may miss early recurrence; ensure MRI uses thin-cut sagittal and coronal views with dynamic contrast if possible; avoid non‑contrast studies unless contraindicated.

Concrete Clinical Example

A 48‑year‑old woman with Cushing’s disease underwent uncomplicated TSS. Post‑op day 6 UFC was normal and morning cortisol 3 µg/dL, confirming remission. At 8 weeks post‑TSS, a contrast‑enhanced pituitary MRI showed no residual adenoma; this scan was stored as the new baseline for annual follow‑up.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
