Determine timing of breast surgery for transgender male

STEP 1 — Gather Information

Collect physical health data: current hormone therapy regimen and duration, chest size and skin elasticity, BMI, smoking status, comorbidities (cardiovascular, diabetes, bleeding disorders), and any prior breast imaging. Assess mental health: persistence of gender dysphoria, stability of psychiatric conditions, capacity for informed consent, psychosocial support, and realistic expectations about surgical outcomes.

STEP 2 — Rule In / Rule Out

Are there uncontrolled medical or mental health conditions that would increase surgical risk (e.g., unstable psychiatric illness, active substance abuse, uncontrolled diabetes, or ongoing tobacco use)? If yes, rule out immediate surgery and address modifiable factors. If no, proceed to assess readiness.

STEP 3 — Classify or Stratify

Classify readiness: (A) Physically and mentally optimized (stable hormone therapy if desired, comorbidities controlled, mental health stable, realistic expectations) → proceed to surgical planning; (B) Needs optimization (e.g., uncontrolled hypertension, pending mental health treatment, smoking) → initiate pre‑operative optimization; (C) Inappropriate due to unrealistic expectations or insufficient psychosocial support → defer surgery and provide counseling.

STEP 4 — Decide

For (A) schedule breast surgery (e.g., mastectomy) after obtaining informed consent and pre‑operative clearance; for (B) initiate targeted interventions (e.g., smoking cessation, medication adjustment, mental health follow‑up) and reassess in 1–3 months; for (C) defer surgery, explore concerns, and consider alternative support or delayed revisit.

Clinical Guardrails / Mimics / Pitfalls

Do not proceed with breast surgery in the presence of active psychosis, suicidality, or untreated severe depression; avoid surgery if patient continues tobacco use or has uncontrolled coagulopathy; do not base timing solely on age, as no specific age threshold is evidence‑based; ensure patient understands that breast surgery does not affect fertility and that chest contouring may require revision surgery.

Concrete Clinical Example

A 24‑year‑old transgender male on testosterone for 14 months reports stable mood, no psychiatric comorbidities, BMI 27, non‑smoker, and desires bilateral mastectomy after 1 year of hormone therapy. Physical exam shows adequate chest tissue. Mental health assessment confirms capacity for consent and realistic expectations. No contraindications identified → schedule surgery after pre‑operative clearance.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
