es-provide-fertility-preservation-counseling

This skill guides clinicians to inform and counsel individuals seeking gender-affirming medical treatment about fertility preservation options before initiating puberty suppression in adolescents or before starting hormonal therapy in adolescents and adults. It is triggered when a patient is eligible for pubertal suppression or sex hormone treatment and is about to begin such therapy.

Provide fertility preservation counseling before gender-affirming treatment

STEP 1 — Gather Information

Collect patient’s assigned sex at birth, gender identity, pubertal status (Tanner stage), mental health assessment confirming capacity to consent, eligibility for pubertal suppression (Tanner G2/B2 or later) or hormonal therapy, desire for biological children, and any medical contraindications to fertility preservation procedures.

STEP 2 — Rule In / Rule Out

Rule in: patient is seeking gender-affirming medical treatment (puberty suppression or sex hormone therapy) and is about to start; rule out: patient not eligible for or not imminently beginning such treatment → defer fertility preservation counseling until eligibility is confirmed.

STEP 3 — Classify or Stratify

Classify by reproductive potential: for patients assigned male at birth, assess feasibility of sperm cryopreservation (post‑pubertal) vs. future spermatogenesis after GnRH analog cessation; for patients assigned female at birth, assess oocyte/embryo cryopreservation (post‑menarche) or experimental ovarian tissue cryopreservation; note that prepubertal patients have limited current options.

STEP 4 — Decide

Provide tailored fertility preservation counseling, discuss success rates, risks, and timing relative to gender‑affirming treatment, refer to reproductive specialist if desired, document the discussion and patient’s decision, then proceed with pubertal suppression or hormonal therapy per the patient’s informed consent.

Clinical Guardrails / Mimics / Pitfalls

Do not assume lack of fertility interest based on gender identity; avoid delaying necessary gender‑affirming care solely for fertility preservation; do not overstate success rates of experimental techniques; remember that GnRH analog suppression is reversible but fertility recovery data are limited; always involve a mental health provider in assent/consent processes for adolescents; do not omit discussion of fertility options when counseling about hormone risks.

Concrete Clinical Example

A 14‑year‑old transgender male (assigned female at birth) at Tanner G2 presents for GnRH analog initiation. Clinician confirms eligibility, discusses that sperm cryopreservation is not feasible pre‑pubertally, explains that spermatogenesis may resume after stopping GnRH analogs (data scarce), refers to reproductive endocrinology for future options, and after shared decision‑making the patient proceeds with pubertal suppression.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
