es-rtcgm-adip-vs-mdis-smbg-t1d

This skill suggests using real-time continuous glucose monitoring (CGM) and algorithm-driven insulin pumps (ADIPs) rather than multiple daily injections (MDIs) with self-monitoring of blood glucose (SMBG) three or more times daily for adults and children with type 1 diabetes (T1D). Trigger phrases include "Should I use RT-CGM+ADIP or MDI+SMBG for this T1D patient?" or "Is intensive pump therapy indicated for this patient?"

RT-CGM+ADIPs vs MDIs+SMBG for Type 1 Diabetes

STEP 1 — Gather Information

Confirm T1D diagnosis, current MDI+SMBG regimen (≥3 checks/day), age, hypoglycemia history, impaired awareness, patient tech readiness, and insurance/device access.

STEP 2 — Rule In / Rule Out

Is the patient with T1D using MDI with SMBG three or more times daily? If yes, proceed; if no, consider alternative glucose monitoring/insulin strategies.

STEP 3 — Classify or Stratify

Stratify by hypoglycemia risk: high (prior severe episodes or impaired awareness) vs lower risk. High-risk patients benefit most; lower-risk patients still considered based on preference and access.

STEP 4 — Decide

For qualifying patients, prescribe RT-CGM+ADIP with structured education; retain SMBG for calibration/confirmation; schedule follow‑up to assess TIR, hypoglycemia, and device tolerance.

Clinical Guardrails / Mimics / Pitfalls

Do not rely solely on CGM without confirmatory SMBG; avoid in patients unable to respond to alerts or manage device wear; do not overlook cost/coverage barriers; never skip diabetes education on device use and hypoglycemia prevention.

Concrete Clinical Example

A 12‑year‑old with T1D on MDI+SMBG 4×/day reports nocturnal hypoglycemia and impaired awareness. After confirming eligibility, RT‑CGM+ADIP is started with training; SMBG used twice daily for calibration. At 3‑month review, TIR improves from 55% to 70% and severe hypoglycemia events drop from 2/month to 0.

概要と使いどころ

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
