esa-pa-test-kcnj5

Determines when to test for germline KCNJ5 mutations indicating familial hyperaldosteronism type III in very young patients with primary aldosteronism. Triggered by severe early-onset hypertension with hypokalemia in infants or young children (<20 years) with confirmed PA.

Determine When to Test for Germline KCNJ5 Mutations (FH-III)

STEP 1 — Gather Information

Confirm PA via elevated ARR and a positive confirmatory test (e.g., saline infusion, fludrocortisone suppression). Collect age, age of hypertension onset, serum potassium, and family history of PA or early stroke (<40 years).

STEP 2 — Rule In / Rule Out

Is the patient <20 years of age with confirmed PA? If yes, proceed to Step 3; if no, do not pursue germline KCNJ5 testing (consider FH-I/FH-II based on family history or routine workup).

STEP 3 — Classify or Stratify

Among patients <20 years with PA, assess for severe early-onset hypertension (SBP >160 mmHg or DBP >100 mmHg) and hypokalemia (serum K+ <3.5 mmol/L). If both are present, classify as high suspicion for FH-III; otherwise, classify as lower suspicion.

STEP 4 — Decide

If high suspicion, suggest germline KCNJ5 testing; if low suspicion, consider alternative diagnoses or proceed with FH-I/FH-II testing based on family history.

Clinical Guardrails / Mimics / Pitfalls

Do not test for germline KCNJ5 in patients >20 years without family history; avoid confusing germline KCNJ5 mutations with somatic KCNJ5 mutations in adenomas; ensure PA is confirmed before genetic testing; refrain from testing in patients with known FH-I or FH-II.

Concrete Clinical Example

A 6‑year‑old boy presents with hypertension 180/110 mmHg, hypokalemia K+ 2.9 mmol/L, elevated ARR, and positive saline suppression test; no family history. He is <20 years with confirmed PA and severe early‑onset hypertension with hypokalemia, prompting germline KCNJ5 testing.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
