jes-pa-mra-normotensive

Recommends mineralocorticoid receptor antagonists for all primary aldosteronism patients to prevent target organ damage, irrespective of blood pressure control or serum potassium levels. Triggered when a clinician encounters a PA patient with well‑controlled BP and normal K and wonders, 'Do I still need to treat with MRA?' or considers stopping therapy.

Determine when to use MRAs in PA patients with controlled blood pressure and normal potassium

STEP 1 — Gather Information

Confirm PA diagnosis via positive screening (ARR ≥200, PAC ≥60 pg/mL) and a confirmatory test; document office/home BP (<130/80 mmHg), serum potassium (3.5‑5.0 mmol/L), current antihypertensives, baseline eGFR and urinalysis.

STEP 2 — Rule In / Rule Out

Is the patient diagnosed with primary aldosteronism?

  • No → Manage hypertension per usual guidelines; MRA not indicated for PA.
  • Yes → Proceed to step 3.

STEP 3 — Classify or Stratify

Assess for contraindications: serum K >5.0 mmol/L, eGFR <30 mL/min/1.73 m², pregnancy (unless spironolactone after first trimester with obstetric advice), or concomitant use of potassium‑sparing agents with eplerenone/esaxerenone.

  • Contraindication present → Avoid MRA; consider alternative antihypertensives or specialist referral.
  • No contraindication → Proceed to step 4.

STEP 4 — Decide

Initiate MRA: spironolactone 25‑50 mg daily, eplerenone 2.5‑5 mg daily, or esaxerenone 1.25‑2.5 mg daily. Re‑check serum potassium, creatinine, and BP at 4‑6 weeks; continue long‑term to prevent target organ damage.

Clinical Guardrails / Mimics / Pitfalls

Do not start MRA if K >5.0 mmol/L or eGFR <30 mL/min/1.73 m²; avoid combining eplerenone/esaxerenone with potassium supplements or potassium‑sparing diuretics; monitor men for gynecomastia with spironolactone; do not discontinue MRA solely because BP and K are well controlled.

Concrete Clinical Example

A 58‑year‑old woman with PA confirmed by AVS, BP 128/76 mmHg on amlodipine 5 mg daily, K 4.2 mmol/L, asks whether she can stop low‑dose spironolactone. She continues MRA; at 4‑week follow‑up K is 4.4 mmol/L, BP stable, no adverse events.

Guide pump setting adjustments for the MiniMed 780G system based on CareLink data, using Medtronic's 3-step follow-up workflow. Use when a clinician has CareLink data for a 780G patient and wants to know what to change — which setting, by how much, and how to program it. Triggers include: "what do I adjust", "how to fix high TIR", "how to reduce hypos", "adjust ICR/ISF/basal", "SmartGuard settings", "780G pump settings review", or any question about optimising 780G therapy after reviewing a download.

Performs corticotropin stimulation test to diagnose adrenal insufficiency when morning cortisol is indeterminate (3-15 µg/dL). Triggers include morning cortisol 3-15 µg/dL requiring further AI testing.

Administers immediate parenteral injection of 50–100 mg hydrocortisone for suspected adrenal crisis secondary to adrenal insufficiency. Triggers include hypotension, hypoglycemia, or acute illness in a patient with known adrenal insufficiency.

Clinicians educate adrenal insufficiency patients taking nondexamethasone glucocorticoids who initiate enzyme-inducing antiepileptic drugs about early signs of adrenal insufficiency. Trigger when an AI patient on nondexamethasone GC starts an enzyme-inducing AED such as phenytoin, carbamazepine, or oxcarbazepine.

In central hypothyroidism patients receiving levothyroxine who initiate enzyme‑inducing antiepileptic drugs, the guideline recommends checking free T4 at least 6 weeks after AED start and increasing the levothyroxine dose if free T4 falls below the target range. Triggers include: central hypothyroidism patient on levothyroxine starting antiepileptic drug such as phenytoin, carbamazepine, or oxcarbazepine.

Applies serum cortisol cutoffs (<3 μg/dL indicates central adrenal insufficiency; >15 μg/dL likely excludes it) when evaluating morning cortisol for suspected adrenal insufficiency. Triggers include morning cortisol measurement in suspected AI and use when interpreting morning cortisol levels for adrenal insufficiency workup.
